Surveys

We really appreciate the time you put into completing the surveys below. As a show of appreciation you will receive a $25 gift card for completing 4 objectives, for a total of $100. Please read under each time point for further details about how to earn your gift card.

Fall

• Fall Teacher Background Questionnaire
  This survey contains questions about your background as a teacher (certifications, experience, etc.) as well as your general classroom demographics.  Please complete this as soon as possible.
• Fall Child Assessment
  On this survey you will enter each child’s PALS assessment scores and answer a few questions about the child. If the child has an IEP or disability, please have their administration file handy so that you can answer these questions appropriately. These surveys can be completed by the lead teacher or the classroom aide.
• Professional Development Surveys
  These 8 surveys contain a few questions concerning your involvement and evaluation of each PD Module and content questions related to the specific PD Module. We recommend completing each survey immediately after finishing the module.

Throughout the Year

• Reading Log
  This survey will be completed after each week’s reading. If you use the spiral bound reading log, please copy your daily notes into this survey at the end of the week.

A $25 Gift card will be given at the end of the year if  90% or more of the weekly reading logs are entered.

Winter

Will be available January 2017

• Professional Development Refresher Survey
  This survey will contain a few questions concerning your PD refresher course. Please take it after finishing the PD.

A $25 Gift card will be given upon completing the refresher PD.

Spring

Will be available April 2017

• Spring Teacher Background Questionnaire
  This survey is very similar to the Fall Teacher Background Questionnaire. It contains questions about your instructional practices, background knowledge, and opinions about your workplace. We are asking that you finish this survey by May 15th.
• Spring Child Assessment
  This survey will be used after reassessing each student using the PALS. We are asking teachers to have all students entered online two weeks before their final day of school.

A $25 Gift card will be given after all Spring Child Assessments are entered online.
